Your first office visit
Before your first office visit:
- Please fill out the "New Patient Registration Form" before your first office visit. You can download this form or fill out online.
- Please read the HIPAA Notice of Privacy Practice.
On your first office visit:
- Please bring your driver license and insurance card.
- If you don’t have driver license please bring a state-issued photo ID.
- Please bring your medical records (for example colonoscopy report, pathology report, CT-scan or MRI report) with you on the appointment day. Please bring your medication list.
- In addition to the new patient form, you may need to fill out and sign other forms in the office.
